ScienceDaily (Sep. 19, 2012) ? Better adherence to the DASH (Dietary Approaches to Stop Hypertension) diet is associated with significant reductions in blood pressure. However, African-Americans may be less likely than whites to adopt the diet, according to researchers at Duke University Medical Center.
The findings, which appear online Sept. 19 in the Journal of the Academy of Nutrition and Dietetics, suggest that altering traditional recipes to meet nutritional guidelines rather than eliminating certain foods altogether may result in better adherence among African-Americans.
The DASH diet is recognized as the diet of choice for preventing and managing high blood pressure. The diet is rich in fruits, vegetables, and low fat dairy products, and is low in fats and cholesterol.
"Previous research, including results from our ENCORE study, established the DASH diet as an important approach for lowering blood pressure, and for some individuals, it may be an effective alternative to taking medication for hypertension," said James A. Blumenthal, PhD, professor of behavioral medicine in the Department of Psychiatry and Behavioral Science at Duke University Medical Center. "In this study we were interested in whether dietary adherence was related to blood pressure changes and what factors predicted who would adhere to the diet."
The study was a new analysis of data from the ENCORE trial, led by Duke researchers to evaluate the effectiveness of the DASH diet on cardiovascular health. Participants were 144 sedentary, overweight or obese adults, who had high blood pressure and were not taking medication.
Researchers measured a series of clinical and behavioral factors at the start of the study including blood pressure, weight, and physical fitness, as well as dietary habits. Depression, anxiety and social support were also evaluated as potential predictors of adherence to the regimen.
Participants were randomly assigned to one of three treatment groups: the DASH diet alone; the DASH diet in combination with weight-loss counseling and aerobic exercise; or no change in diet and exercise habits.
After four months, participants in the group that got the DASH diet plus weight-loss counseling and exercise lost an average of 19 pounds, while weight remained stable in the other two groups.
Participants in both the DASH diet alone and DASH diet plus counseling groups had significant reductions in blood pressure, with greater adherence to the DASH diet resulting in the largest drops in blood pressure. The finding suggests that that following the DASH diet lowers blood pressure, independent of exercise and weight loss.
However, the addition of weight loss and exercise to the DASH diet promoted even greater reductions in blood pressure and improved other measures of cardiovascular health. "For overweight or obese patients with high blood pressure, clinicians should recommend the DASH diet in conjunction with exercise and weight loss for the best results," said Alan Hinderliter, MD, a cardiologist at the University of North Carolina at Chapel Hill and an investigator in this study.
The researchers noted that African-American participants were less likely than white participants to eat foods recommended in the DASH diet prior to beginning the study. While both African-American and white participants in the DASH treatment groups increased the amount of DASH foods they ate, African-Americans were less likely to adopt the DASH diet compared to their white counterparts. No other demographic, behavioral, or social variable predicted whether participants would adhere to the DASH diet.
"We need to be aware of cultural differences in dietary preferences in order to help people better adopt a DASH-friendly diet," Blumenthal said. "It is important to take into account traditional food choices and cooking practices when attempting to incorporate more DASH foods into daily meal plans."
Culturally sensitive changes to implementing the DASH diet, such as modifying traditional "soul food" recipes to meet nutritional recommendations rather than eliminating foods altogether, may result in better adherence among African-Americans.
"Given the success of the DASH diet, we know that changing lifestyles can make a significant difference in people's health," Blumenthal said. "And in the long run, if people are able to maintain changes to their diet and exercise habits, it can lead to a lower risk for heart attack and stroke."
In addition to Blumenthal, Duke researchers include Dawn Epstein, Andrew Sherwood, Patrick J. Smith, Carla Caccia, Pao-Hwa Lin, Michael A. Babyak, and Julie J. Johnson. Other researchers include Linda Craighead of Emory University, and Alan Hinderliter of the University of North Carolina at Chapel Hill.
The study was funded with grants from the National Heart, Lung, and Blood Institute (HL074103), and the General Clinical Research Center, National Institutes of Health (M01-RR-30).

"The Queen of Versailles" is funny, sad, infuriating, instructive. It's the American Dream inflated to ridiculous extremes, until it bursts.
More than a social morality tale, this is a character study, with the title well chosen.
"The Queen of Versailles" ought to be required viewing for anyone who blames the rich for yanking the rug out from under America's economy.
By the end, the movie has pulled off a small miracle: You become absorbed in the lives of these people for who they are and not what they own.

(Reuters) - A U.S. regulation that limits how much of its own capital a bank can put at risk is causing headaches for Morgan Stanley as it prepares to raise a new multi-billion-dollar global infrastructure fund, people familiar with the situation said.
The regulation, called the Volcker rule, puts a cap on the amount of capital that Morgan Stanley can pledge to the new fund. That means senior executives at Morgan Stanley Infrastructure Partners will have to make do with a smaller share of the fund's profits, the sources said.
While the majority of the executives have so far accepted the new reality and Morgan Stanley is in talks to increase the fund managers' share of profits, a few have left the bank, the sources said, declining to be identified because they were not authorized to speak publicly on the matter.
Over the past year, at least four key executives who oversaw infrastructure fund investments in the Americas, Europe and Asia have left or are in the process of leaving the Wall Street firm, the sources said.
The most recent departures were Adil Rahmathulla, an executive director for investments in the Americas, and Gautam Bhandari, a managing director and head of Morgan Stanley Infrastructure in Asia, the sources said.
Morgan Stanley spokesman Matt Burkhard declined to comment and would not make executives available for comment.
Morgan Stanley's problems underscore the disadvantage banks have when competing with independent alternative asset managers such as Blackstone Group LP and Carlyle Group LP. They also highlight the constraints facing U.S. investment banks wishing to hold on to traditionally strong but capital-intensive businesses, particularly private equity assets that, by nature, are usually tied up for several years.
Infrastructure funds invest in assets such as roads, airports, power grids and electricity transmission networks. They offer investors low-risk returns with a long-investment horizon. But being private equity-type assets, they are much less liquid than traditional stocks and bonds.
The future of such businesses at banks like Morgan Stanley and Goldman Sachs Group Inc has been in question since the Volcker rule passed in 2010. The rule, which was mandated by the 2010 Dodd-Frank Act and named for former Federal Reserve Chairman Paul Volcker, is expected to be finalized by the end of this year.
Morgan Stanley's current $4 billion infrastructure fund, launched in 2008, is part of the bank's merchant banking business, which in turn is housed inside its investment management division.
The infrastructure fund has about 40 employees, with roughly 10 in senior investment roles, sources said.
Investment banks are already having a hard time convincing investors that they are the right custodians of alternative assets. Goldman Sachs was not as successful in its second infrastructure fund endeavor, slashing its fundraising target in half in the middle of its marketing process and ending up raising $3.1 billion.
Banks have been approaching the regulatory hurdle with different strategies. Bank of America Corp, HSBC Holdings PLC and Credit Suisse Group AG have each outlined plans to exit some private-equity businesses. Deutsche Bank AG had planned to do sell some of its alternative asset businesses as well, but reversed course in a strategic overhaul last week.
VOLCKER CHANGES
Under the Volcker rule, Morgan Stanley will have to cap investments in new private equity funds at 3 percent, far below the 10 percent investment it contributed to its existing infrastructure fund.
Fund managers have decided to increase their personal commitment to the new fund, targeting about 1 percent of its capital compared to 0.5 percent of capital of the 2008 fund, according to one of the sources.
Some executives want the firm to do more to compensate fund managers for losing out on profits as a result of Morgan Stanley's smaller commitment, with one of them even calling for a spin out of the infrastructure business, the sources said.
Morgan Stanley has been offering to share more of the carried interest - its slice of investment profits - with the fund managers. Fund managers currently get 60 percent of the carried interest, up from an initial level of 50 percent, one source said. They are negotiating a further increase for the new fund, the source added.
To be sure, a lower contribution from Morgan Stanley into the new infrastructure fund may not affect investor sentiment and the amount of money the bank is able to eventually raise.
For example, Global Infrastructure Partners (GIP), an independent firm, is putting less than 1 percent of its own money into its latest $8.25 billion infrastructure fund, according to a person familiar with the matter.
GIP did not respond to a request for comment.
DEPARTURES
But the promise of higher pay elsewhere has played a role in at least some Morgan Stanley fund executives looking for opportunities outside the bank, the sources said. Independent firms, such as GIP and Alinda Capital Partners, do not have a bank owner they have to share profits with.
Of the recent departures, Rahmathulla is on "gardening leave" - or the notice period when he is still on payroll - considering opportunities at other independent funds, sources said.
Bhandari is still at Morgan Stanley but recently informed limited partners of his intention to leave and may eventually launch his own fund, sources said.
Morgan Stanley is speaking to him about the possibility of launching an emerging markets-focused fund off its infrastructure platform, one of the sources added.
Other executives who have left the fund in the past year include Sadek Wahba, the former chief investment officer who left the fund in late 2011; and Vincent Policard, who oversaw some investments in Europe and quit Morgan Stanley in August 2011. Private equity firm KKR & Co announced in February that it had hired him.
Wahba and Policard could not be reached for comment.
After Wahba left, Morgan Stanley named three co-heads of the business - Markus Hottenrott, who is also chief investment officer; Anne Valentine Andrews, who is also chief operating officer; and Jim Wilmott, who is also head of Europe.
John Veech, a managing director, oversees investments in the Americas division where Rahmathulla worked.

Power, denied for a third consecutive year, was in street clothes watching the finish on television after crashing out early at Auto Club Speedway. He took a 17-point lead into the finale, but once out of the race, gave Hunter-Reay total control of his destiny.
But it certainly wasn't easy.
Hunter-Reay struggled all week at California, even wrecking in Wednesday's open test session, and was off at the start of the race. But he picked his way through the field and was on the edge of where he needed to finish as the laps wound down.

"It was really an unbelievable weekend," said Andretti, who won his fourth championship as an owner but first since 2007.
Power, meanwhile, was left wondering how yet another title slipped away.
He's the most dominant driver on road and street courses, and knows that racing on ovals is the one glaring weakness in his program. And when he looks back at this season, he knows that a blocking penalty at Texas and crashes on the ovals at Indianapolis, Iowa, and now, California, cost him the title.
"I wish I could put my finger on one particular thing I don't do right," Power said. "If you look at it, three ovals I crashed out of is a massive hit on points. It's over a whole season that you win a championship and that was proven tonight."
Power crashed 55 laps into the race, spinning hard into the outside wall when his car slipped in a seam in the track. It's the third consecutive year Power has gone into the finale with the title on the line and had an incident snatch away his chances.
He brushed the wall at Homestead in 2011 and lost the title by five points to Dario Franchitti. Last year, his points lead was gobbled up when another car hit him on pit road in the penultimate race, and he was involved in the 15-car accident that killed Dan Wheldon in the finale.
Power broke his back in that accident in Las Vegas.
"Man, depressing," Power said after leaving the wreck. "I wish I could care less."
Power laughed nervously, searching and failing to find the right words to describe his frustration.
"I don't know what to say. It's depressing," he said. "Depressing to lose the championship again that way. Nothing I can say, mate, it's just depressing. I don't know what emotion to even feel right now."
But his Penske team, which has not won a championship since Hornish in 2006, worked furiously to get his car back on the track so he could turn 12 laps and gain more points in the standings. Power had already changed into street clothes, rushed back into his firesuit, and had a total change of attitude when he got back in the car.
"Keep our fingers crossed," he smiled, his mood completely changed.
Both Power and Hunter-Reay went into the race seeking their first career title, and Power said he watched on TV the closing laps unsure how it would end. He was cracking self-deprecating jokes when it was over, perhaps an unusual reaction for one of the most intense and driven drivers in the series.
"Well, I've done it for two years in a row, so I've kind of learned how to cope with it," he said.
